To demonstrate the feasibility of extending electronic health record (EHR) data for analysis of community-based health care and describe barriers to the use of community EHR data. A retrospective review was conducted of the Wisconsin Disease Reidentification Program (DReIP), a voluntary state-wide disease registry using an EHR. Descriptive statistics were used to report data on disease reidentification among community members in the DReIP. A total of 19,636 patients had at least one outpatient visit in Wisconsin during 2005-2010. From 2005-2010, there were 1,109,493 inpatient stays, 12,127,926 outpatient visits and 228,819 emergency department (ED) visits in Wisconsin. The frequency of disease reidentification (of record in the EHR) among the 925 patients in the DReIP who had at least one hospitalization in Wisconsin was 15.8%. The frequency of reidentification among the 1,109,493 patients who had at least one inpatient visit in Wisconsin was 3.4%. Only the data from ED visits in Wisconsin were consistently available for disease reidentification. While the ED population is likely to be the most difficult population to reidentify because of the large number of visits, the frequency of reidentification was lower than the inpatient and outpatient populations (2.5% vs. 4.3% and 4.0%, respectively).
